ABSTRACT

Introduction. Brazil's health system offers insights into addressing the double burden of malnutrition by proper population monitoring, coupled with local policies and national guidelines. Objective. To investigate the recent temporal trends in nutritional status indicators and its coverage of children aged two to five years from Campinas, a metropolis in the state of São Paulo, Brazil. Material and methods. The analysis of time series from 2018 to 2022 were conducted by accessing data from the Brazilian Food and Nutrition Surveillance System (SISVAN) due to regression analysis. Results. The number of children aged two to five in the SISVAN registry rose from 7,300 in 2018 to 11,171 in 2022, forming the study sample. In 2018, 700 were chronically undernourished, 306 underweight, and 977 overweight; by 2022, 530 showed stunting, 457 were underweight, and 1,084 overweight. Stunting prevalence declined from 9.6% (2018) to 4.7% (2022). Underweight dropped slightly from 4.2% to 4.1% over the years and overweight, consistently the highest indicator, ranged from 13.4% (2018) to 9.7% (2022). SISVAN's coverage varied between 16.6% (2018) and 26.1% (2022), the lowest at 12.3% in 2020. The trend for stunting decreased significantly (APV: -15.01; CI95% -22.64; -6.62). Adding SISVAN's coverage variable the model slightly reduced the declining stunting trend (APV: -12.12; CI95% -13.19; -11.04). Conclusions. Coordinated efforts to address nutritional challenges, from adequate population monitoring to the interaction between local policies and national guidelines, have shown positive health outcomes(AU)


Introducción. El sistema de salud de Brasil ofrece conocimientos valiosos para abordar la doble carga de malnutrición mediante el seguimiento adecuado de la población y políticas locales y nacionales. Objetivo. Investigar las tendencias temporales recientes en indicadores nutricionales y cobertura en niños de dos a cinco años en Campinas, São Paulo, Brasil. Materiales y métodos. Se analizaron datos del SISVAN de 2018 a 2022 con regresión temporal. Resultados. Niños de dos a cinco años en el SISVAN pasaron de 7,300 en 2018 a 11,171 en 2022. En 2018, 700 estaban crónicamente desnutridos, 306 con bajo peso y 977 con sobrepeso; en 2022, 530 con retraso en el crecimiento, 457 con bajo peso y 1,084 con sobrepeso. La prevalencia del retraso en el crecimiento bajó de 9,6% a 4,7%. El bajo peso disminuyó levemente del 4,2% al 4,1% y el sobrepeso osciló del 13,4% al 9,7%. La cobertura de SISVAN varió de 16.6% a 26.1%, con un mínimo de 12.3% en 2020. La tendencia del retraso en el crecimiento disminuyó significativamente (APV: -15.01; CI95% -22.64; -6.62). La inclusión de la cobertura de SISVAN redujo levemente esta tendencia (APV: -12.12; CI95% -13.19; -11.04). Conclusiones. Coordinar esfuerzos para abordar los desafíos nutricionales, desde el seguimiento de la población hasta políticas locales y nacionales, ha tenido resultados positivos para la salud(AU)

ABSTRACT

As interações sociais iniciais são fundamentais para o desenvolvimento infantil, sendo essencial o papel dos adultos na promoção de práticas que o favoreçam. Tais práticas podem ser influenciadas por aspectos contextuais e pelas emoções, concepções e expectativas parentais sobre seus filhos. No presente estudo, foi considerada a configuração familiar caracterizada pela presença de bebês gêmeos. O objetivo foi conhecer as emoções das mães acerca da gravidez gemelar e analisar as suas concepções e expectativas sobre o desenvolvimento dos gêmeos nos primeiros 24 meses de vida. Participaram deste estudo nove mães de bebês gemelares, que responderam a um questionário sociodemográfico e a uma entrevista semiestruturada. Os resultados revelaram sentimentos maternos ambivalentes relativos à experiência com a gemelaridade desde a gestação; concepções sobre desenvolvimento infantil, fatores que podem influenciá-lo e diferenças entre os gêmeos; e expectativas sobre o desenvolvimento dos bebês. Conclui-se que uma maior compreensão sobre aspectos relacionados à maternidade de gemelares pode auxiliar na criação de redes de apoio socioemocional aos cuidadores e de contextos que favoreçam as interações estabelecidas entre a tríade mãe-bebês.


Initial social interactions are fundamental to child development, and the role of adults in promoting practices that favor them is essential. Such practices can be influenced by contextual aspects and by parental emotions, conceptions and expectations about their children. In the present study, the family configuration characterized by the presence of twin babies was considered. The objective was to get to know the mothers' emotions about twin pregnancies and to analyze their conceptions and expectations about the development of twins in the first 24 months of life. Nine mothers of twin babies participated in this study, who answered a sociodemographic questionnaire and a semi-structured interview. The results revealed ambivalent maternal feelings regarding the experience with twinning since pregnancy; conceptions about child development, factors that can influence it and differences between twins; and expectations about the development of babies. We conclude that a greater understanding of aspects related to twin maternity can help in the creation of social and emotional support networks for caregivers and contexts that favor the interactions established between the mother-babies triad.


Las interacciones sociales iniciales son fundamentales para el desarrollo infantil, siendo fundamental el rol de los adultos en la promoción de prácticas que los favorezcan. Dichas prácticas pueden estar influenciadas por aspectos contextuales y por emociones, concepciones y expectativas de los padres sobre sus hijos. En el presente estudio se consideró la configuración familiar caracterizada por la presencia de bebés gemelos. El objetivo fue conocer las emociones de las madres sobre los embarazos gemelares y analizar sus concepciones y expectativas sobre el desarrollo de los gemelos en los primeros 24 meses de vida. En este estudio participaron nueve madres de gemelos, quienes respondieron un cuestionario sociodemográfico y una entrevista semiestructurada. Los resultados revelaron sentimientos maternos ambivalentes con respecto a la experiencia del hermanamiento desde el embarazo; concepciones sobre el desarrollo infantil, factores que pueden influir en él y diferencias entre gemelos; y expectativas sobre el desarrollo de los bebés. Concluimos que una mayor comprensión de los aspectos relacionados con la maternidad gemelar puede ayudar en la creación de redes de apoyo social y emocional a los cuidadores y contextos que favorezcan las interacciones que se establecen entre la tríada madre-bebé.

ABSTRACT

Objective:To study the neurodevelopmental outcomes and risk factors of premature infants with gestational age (GA) <34 weeks.Methods:From June 2016 to June 2018, premature infants with GA<34 weeks admitted to the Neonatology Department of our hospital were retrospectively reviewed. Bayley Scales of Infant DevelopmentⅡwas used to assess the neurodevelopmental outcomes at corrected GA 18~24 months. The incidence of neurodevelopmental impairment (NDI) was determined. Mental developmental index (MDI) and psychomotor developmental index (PDI) were calculated. The infants were assigned into three groups according to their MDI and PDI scores:≥85 group, 70~84 group and <70 group. Risk factors of low MDI and PDI scores were analyzed.Results:A total of 202 premature infants with GA<34 weeks were included, including 131 males (64.9%) and 71 females (35.1%). 91 cases (45.0%) developed NDI, including 77 mild NDI (38.1%) and 14 severe NDI (6.9%). Univariate analysis found that the incidences of severe asphyxia, multiple births, Grad≥3 ROP and endotracheal intubation in the MDI<70 group were higher. At corrected GA 40 weeks, 3, 6, 12 months, MDI<70 group showed less cases of head circumference >-2SD. PDI<70 group had higher incidences of intrauterine distress, maternal gestational hypertension, multiple birth, Grade 3~4 RDS, Grade 3~4 germinal matrix-intraventricular hemorrhage and tracheal intubation ( P<0.05). Logistic regression showed gestational hypertension, history of tracheal intubation and invasive ventilation were the risk factors for low PDI score ( OR=9.176, 4.547 and 3.227, P<0.05). The head circumference >-2SD at corrected age 6m was protective factor for low MDI and PDI scores ( OR=0.063 and 0.041, P<0.001). Conclusions:Preterm infants with GA<34 weeks are likely to develop severe NDI. Avoiding unnecessary tracheal intubation and invasive ventilation and improving gestational hypertension management may be beneficial to the neurodevelopmental outcomes of preterm infants.

ABSTRACT

La displasia del desarrollo de la cadera comprende un conjunto de anormalidades que afectan la articulación coxofemoral: la displasia, subluxación y luxación de la cadera. El concepto de "displasia" describe anormalidades en la estructura femoral, acetabular o ambas. Corresponde a la patología ortopédica más frecuente del recién nacido y lactante, lo que genera mucha preocupación e intranquilidad entre los padres, en los primeros controles sanos de sus hijos. Es una patología en la que un diagnóstico oportuno y precoz son la clave para poder realizar un tratamiento efectivo, obteniendo como resultado una cadera clínica y radiológicamente normal al finalizar el desarrollo esquelético. Para esto es fundamental conocer la patología e ir activamente en su búsqueda. Actualmente existe mucha discusión sobre la manera de pesquisar esta patología. En Chile, se realiza tamizaje universal con imagen -radiografía de pelvis- a todos los niños a los 3 meses de edad. El objetivo de la siguiente revisión, es traer a la práctica clínica actual de todos aquellos profesionales que se enfrentan en distintos escenarios a esta patología: médicos de atención primaria, enfermeras, médicos en etapas de destinación y formación en distintas regiones del país, pediatras y ortopedistas, aquellas características y signos de sospecha propios de esta enfermedad y detallar las herramientas para un correcto diagnóstico y oportuno tratamiento.


Developmental dysplasia of the hip (DDH) comprises a set of abnormalities that affect the hip joint: hip dysplasia, subluxation, and dislocation. It is the most frequent orthopedic pathology of the newborn and infant, and it generates great concern among parents during the first health check-ups of their children. It is a condition in which a timely and early diagnosis is key to be able to carry out an effective treatment, obtaining as a result of a clinically and radiologically normal hip at the end of skeletal development. For this, it is essential to know this orthopedic condition and actively search for it. Currently, there is much discussion about how to screen DDH. In Chile, universal screening with imaging - pelvic radiography - is performed on all children at 3 months of age. The objective of the following review is to bring to the current clinical practice of all those professionals who face this pathology in different scenarios: primary care physicians, nurses, physicians in training stages in different regions of the country, pediatricians and orthopedic surgeons, signs of suspicion typical of the disease and detail the assessment tools for a correct diagnosis and timely treatment.

ABSTRACT

Este artigo analisou concepções parentais sobre as características infantis que mais chamam a atenção dos pais, aquelas com as quais eles têm mais dificuldades de lidar e os fatores aos quais essas características são atribuídas. Participaram 20 casais, entrevistados aos 3, 6, 9 e 12 meses dos bebês. Os dados foram analisados qualitativamente. As habilidades socioemocionais foram as características que mais chamaram a atenção. Como principais dificuldades, as mães mencionaram lidar com a teimosia infantil e os pais referiram dificuldades em lidar com o choro dos bebês. Entre os fatores aos quais essas características são atribuídas, foram citados aspectos biológicos e ambientais. Conhecer as concepções parentais sobre desenvolvimento contribui na fundamentação de programas de orientação para profissionais e pais em diferentes áreas.


This article analyzed parental conceptions about child characteristics that most call parents' attention, those which they have more difficulties to deal with and the factors to which these characteristics are attributed. Twenty couples participated in the study, interviewed at the babies' 3rd, 6th, 9th and 12th months. The data were analyzed qualitatively. Socio-emotional skills were the characteristics that most attracted attention. As main difficulties, mothers mentioned dealing with infant stubbornness and fathers reported difficulties in dealing with babies' crying. Among the factors to which these characteristics are attributed, biological and environmental aspects were mentioned. Knowing parental conceptions about development contributes to the foundation of guidance programs for professionals and parents in different areas.


Este artículo analizó las concepciones parentales sobre las características del niño que más llaman la atención de los padres, aquellas con las que tienen más dificultades para tratar y los factores a los que se atribuyen estas características. Participaron del estudio veinte parejas, entrevistadas a los 3, 6, 9 y 12 meses de vida de los bebés. Los datos fueron analizados cualitativamente. Las habilidades socioemocionales fueron las características que más llamaron la atención. Como dificultades principales, las madres mencionaron tener que tratar con la terquedad infantil y los padres informaron dificultades para lidiar con el llanto de los bebés. Entre los factores a los que se atribuyen estas características, se mencionaron aspectos biológicos y ambientales. Conocer las concepciones de los padres sobre el desarrollo contribuye a la fundamentación de programas de orientación para profesionales y padres en diferentes áreas.

ABSTRACT

O objetivo deste estudo foi avaliar a influência terapêutica dos dispositivos orais Hyrax e Bionator de Balters no tratamento da Síndrome da Apneia Obstrutiva do Sono (SAOS), sobre a melhora da SAOS observado pela polissonografia, atividade elétrica dos músculos masseter e temporal, força máxima de mordida, qualidade de vida, e diâmetro dos pontos cefalométricos. Foram selecionados 11 participantes de ambos os sexos, diagnosticados com SAOS através do exame polissonográfico tipo III e com necessidade de tratamento ortopédico facial. Foi feita a coleta dos dados, os responsáveis preencheram os questionários (Escala de Distúrbios do Sono em Crianças e OSA-18-PV), termo de consentimento livre e esclarecido aos responsáveis e para as crianças, documentação ortodôntica (incluindo a análise do diâmetro dos pontos cefalométricos - espaço nasofaríngeo anterior e posterior (NFA-NFP), assim como espaço bucofaríngeo anterior e posterior (BFA-BFP)), análises iniciais da eletromiografia dos músculos masseter e temporal e força máxima de mordida. Estas informações foram coletadas no início do tratamento e após 11 meses, assim como o exame polissonográfico tipo III. Os dados foram normalizados pelo apertamento com Parafilm M e submetidos à análise normalidade pelo teste de Kolmogorov-Smirnov, sendo observado os dados: valor de polissonografia; eletromiografia de masséter direito e esquerdo; eletromiografia de temporal direito e esquerdo; força de mordida em região de incisivo e molar esquerdo/direito, os questionários foram avaliados através de score e nota (0-10), e cefalometria (análise das distâncias NFA-NFP e BFABFP). Para estes dados, foi realizado o teste t de Student. Para os dados com distribuição não normal, foi realizado o teste de Wilcoxon. Todas as análises foram realizadas com nível de significância de 5%. Em relação ao exame polissonográfico e os questionários, houve diferença estatística evidenciando melhora na qualidade do sono dos indivíduos. A eletromiografia demonstrou que houve diferença estatística em repouso no músculo temporal direito, e na mastigação de uva passas, nos músculos masseter e temporal do lado direito. Na cefalometria, foi observada diferença estatística na distância entre os pontos NFA-NFP. Conclui-se que o uso dos dispositivos Hyrax e Bionator de Balters em crianças classe II e portadoras da SAOS é uma alternativa de tratamento segura e eficaz(AU)


The aim of the study was to evaluate the influence of oral devices Hyrax and Balters Bionator in the treatment of Obstructive Sleep Apnea Syndrome (OSAS), on the improvement of OSAS observed by polysomnography, electrical activity of the masseter and temporal muscles, maximum bite force, quality of life, and diameter of cephalometric points. Eleven participants of both genders, diagnosed with OSAS through type III polysomnographic examination and in need of facial orthopedic treatment, were selected. Data collection was performed, the guardians filled out the questionnaires (Sleep Disorders Scale in Children and OSA-18-PV), informed consent form to guardians and for children, cephalometric orthodontic documents - nasopharyngeal space anterior and posterior (NFA-NFP), as well as anterior and posterior buccopharyngeal space (BFA-BFP)), initial analysis of the electromyography of the masseter and temporal muscles and maximum bite force. This information was collected at the beginning of treatment and after 11 months, as well as the type III polysomnographic exam. Data were normalized by tightening with Parafilm M and analysis of normality using the Kolmogorov-Smirnov test, observing the following data: polysomnography value; right and left masseter electromyography; right and left temporal electromyography; bite force in the left/right incisor and molar region, the questionnaires were obtained through score and grade (0-10), and cephalometry (analysis of the NFA-NFP and BFA-BFP distances). For these data, the Student test was performed. For data with non-normal distribution, the Wilcoxon test was performed. All analyzes were performed with a significance level of 5%. Regarding the polysomnographic exam and the questionnaires, there was a statistical difference showing an improvement in the sleep quality of the respondents. The electromyography required that there was a statistical difference at rest in the right muscle, and when chewing raisins, in the masseter and temporal muscles on the right side. In cephalometry, a statistical difference was observed in the distance between the NFA-NFP points. It can be concluded that the use of the Balters Hyrax and Bionator devices in class II children and children with OSAS is a safe and effective treatment alternative(AU)

ABSTRACT

ABSTRACT Purpose: to evaluate the cognitive, language, and motor development of infants with congenital syphilis in their first months of life. Methods: a sample of 52 infants, from 21 to 112 days of age, born in public maternity hospitals, divided into a study group of 28 infants with congenital syphilis and a control group 24 infants without risk indicators for hearing loss. They underwent the Neonatal Hearing Screening Protocol with automated Brainstem Auditory Evoked Potential. The research instrument was the Bayley-III Scale, consisting of the cognitive, language (receptive and expressive), and motor (fine and gross) subscales. The raw scores were entered into the software that accompanies the Bayley-III scale kit to calculate the scores (scaled and composite) and perform qualitative analysis. The Mann-Whitney test and the Wilcoxon test was applied and used a 5% significance level. Results: a similarity between groups for the demographic parameters, maternal education level, and socioeconomic level, was seen. There was no statistically significant difference between groups when comparing the cognitive, receptive language, expressive language, fine motor, and gross motor subscales. Conclusion: infants with congenital syphilis treated at birth have a cognitive, language, and motor development within that expected for their age group in their first months of life.

ABSTRACT

Abstract Quality of Life (QoL) is a predictor of development depending on multiple factors, being the QoL of infants still little studied, especially in permanent settings such as infants educational center or daycare centers. Correlate quality of life with age, family income and the neuropsychomotor development of infants between 4 and 18 months of age in daycare centers. Descriptive transverse study research, with clinical trials registration : RBR 2hd6sm on November 2, 2016. Quality of life was evaluated with interviews with the family through Pediatric Quality of Life Inventory-PedsQL™ (Brazilian version). The infants were evaluated in a playful way, through the use of Alberta Infant Motor Scale (AIMS) and Denver II test . 88 infants participated in the study. The infants evaluated had a good quality of life, with scores above 64%. AIMS presented the correlation with for physical functioning and total score of QoL. QoL of infants from 4 to 18 months of age is correlated with their neuropsychomotor development, which suggests the need for investigations between this theme and daycare centers.


Resumo Qualidade de vida (QV) é um preditor de desenvolvimento e depende de múltiplos fatores, sendo a QV de bebês ainda pouco estudada, especialmente em ambientes de permanência como os centros de educação infantil ou creches. Correlacionar a qualidade de vida com idade, renda familiar e desenvolvimento neuropsicomotor de bebês entre 4 a 18 meses de idade que frequentam centros de educação infantil. Pesquisa descritiva transversal, com registro clínico RBR 2hd6sm em novembro de 2016. A Qualidade de Vida foi avaliada por meio de entrevistas com as famílias com o questionário Pediátrico de Qualidade de Vida-PedsQL™ (versão brasileira). Os bebês foram avaliados de maneira lúdica pela escala motora infantil de Alberta (AIMS) e pela Denver II. 88 bebês participaram do estudo. Os bebês avaliados tiveram uma boa qualidade de vida com escores acima de 64%. AIMS apresentou correlação com os escores de capacidade física e escore total de qualidade de vida. Qualidade de vida de bebês de 4 a 18 meses de idade correlacionam-se com seu desenvolvimento neuropsicomotor, sugerindo a necessidade de investigações sobre esse tema em centros de educação infantil.

ABSTRACT Objective: To know the evidence available in the literature on the effects of the zika virus in children development after fetal exposure. Methods: This is an integrative literature review with 16 scientific articles found in five databases (PubMed, LILACS, CINAHL, Web of Science and Scopus), based on the guiding question: "What are the effects in the development of children aged 0 to 6 years exposed to the zika virus in the fetal period? The STROBE statement was used for data extraction and evaluation of primary studies. Results: Exposure to the zika virus in the fetal period resulted in several congenital anomalies and/or changes in the central nervous system: microcephaly, ocular problems, neurosensorial problems, ventriculomegaly, intracranial calcification, cardiopathy, arthrogryposis, among others. Conclusion: The zika virus is neurotropic; its effect in the fetal nervous system causes irreparable damage to the child, so health professionals, especially nurses, must intensify maternal and also childcare.


RESUMEN Objetivo: Conocer las evidencias disponibles en la literatura sobre los efectos del virus zika en el desarrollo infantil postexposición en el período fetal. Método: Se trata de una revisión integrativa de la literatura con 16 artículos científicos encontrados en cinco bases de datos (PubMed, LILACS, CINAHL, Web of Science y Scopus), basada en la interrogación: "¿Cuáles los efectos en el desarrollo de niños de 0 a 6 años expuestos al virus zika en el período fetal?" Para extracción de datos y evaluación de estudios primarios, se utilizó la declaración STROBE. Resultados: La exposición al virus zika en el período fetal resultó en diversas anomalías congénitas y/o alteraciones en sistema nervioso central: microcefalia, problemas oculares, neurosensoriales, ventriculomegalia, calcificación intracraneal, cardiopatía, artrogriposis, etcétera. Conclusión: El virus zika es neurotrópico; su efecto en el producto fetal causa daños irreparables al niño, luego profesionales de salud, especialmente los enfermeros, deben intensificar el cuidado materno-infantil.


RESUMO Objetivo: Conhecer as evidências disponíveis na literatura sobre os efeitos do vírus zika no desenvolvimento infantil pós-exposição no período fetal. Métodos: Trata-se de uma revisão integrativa da literatura com 16 artigos científicos encontrados em cinco bases de dados (PubMed, LILACS, CINAHL, Web of Science e Scopus), com base na pergunta norteadora: "Quais os efeitos no desenvolvimento de crianças de 0 a 6 anos de idade expostas ao vírus zika no período fetal?" Para a extração dos dados e avaliação dos estudos primários, utilizou-se a declaração STROBE. Resultados: A exposição ao vírus zika no período fetal resultou em diversas anomalias congênitas e/ou alterações no sistema nervoso central: microcefalia, problemas oculares, neurosensoriais, ventriculomegalia, calcificação intracraniana, cardiopatia, artrogripose, dentre outras. Conclusão: O vírus zika é neurotrópico; seu efeito no produto fetal causa danos irreparáveis à criança, portanto profissionais da saúde, em especial os enfermeiros, devem intensificar o cuidado materno-infantil.

Introducción: La primera infancia es el período de mayor neuroplasticidad. La detección temprana de alteraciones del desarrollo permite la intervención oportuna para reducir secuelas y/o complicaciones. Objetivo: Evaluar el desarrollo psicomotor y cognitivo en lactantes asistidos en el sector público de salud, entre los 6 y 9 meses de edad. Población y métodos: Estudio descriptivo de seguimiento, de niños nacidos a término, clínicamente sanos en la evaluación, con peso, talla y perímetro cefálico adecuados para la edad. Se utilizó la Escala de Evaluación del Desarrollo Psicomotor y la Escala Argentina de Inteligencia Sensorio-motriz entre los 6 y 9 meses. En los casos en que se detectaron riesgo o retraso, se intervino con estimulación temprana. Se determinaron las prevalencias de riesgo/retraso del desarrollo psicomotor e inteligencia sensorio-motriz. Se utilizó el paquete estadístico R, versión 3.3.1. Resultados: Fueron evaluados 102 niños. A los 6 meses, el 22,5 % presentó riesgo/retraso en el desarrollo. Las prevalencias de resultados no esperados para la edad en desarrollo psicomotor y en inteligencia sensorio motriz fueron del 13,7 % y del 16,7 %, respectivamente. Estos niños recibieron estimulación temprana y todos mejoraron sus resultados. A los 9 meses, la prevalencia de riesgo/retraso fue del 20,6 %: el 14,7 % en desarrollo psicomotor y el 9,8 % en inteligencia sensorio motriz. Conclusión: El 22,5 % de los lactantes presentó riesgo/retraso del desarrollo a los 6 meses; la prevalencia a los 9 meses fue del 20,6 %. La evaluación temprana del desarrollo permitió detectar alteraciones e intervenir tempranamente.


Introduction: Infancy is the period of greatest neuroplasticity. The early detection of developmental disorders helps to provide a timely intervention aimed at reducing sequelae and/or complications. Objective: To assess psychomotor and cognitive development in infants between 6 and 9 months old assisted in the public health sector. Population and methods: Descriptive study with follow-up of full-term infants who were clinically healthy at the time of assessment and had an adequate weight, height, and head circumference for their age. The Assessment Scale of Psychomotor Development and the Argentine Scale of Sensorimotor Intelligence were used in infants aged 6-9 months. Early stimulation was the intervention provided to those who showed a risk or delay. The prevalence of risk/delay of psychomotor development and sensorimotor intelligence was determined. The statistical software package R, version 3.3.1 was used. Results: A total of 102 children were assessed. At 6 months old, 22.5 % had a developmental risk/delay. The prevalence of unexpected results for age in psychomotor development and sensorimotor intelligence was 13.7 % and 16.7 %, respectively. These children received early stimulation and all showed improved results. At 9 months old, the prevalence of risk/delay was 20.6 %: 14.7 % in psychomotor development and 9.8 % in sensorimotor intelligence. Conclusion: At 6 months old, 22.5 % of infants had a developmental risk/delay; at 9 months old, this prevalence was 20.6 %. An early assessment of development helped to detect disorders and provide an early intervention.

RESUMO Buscou-se investigar o impacto da creche na interação mãe-criança e no desenvolvimento infantil nos dois primeiros anos de vida da criança. Participaram 44 crianças (M= 6,4 meses; DP= 1,6) e suas mães (M= 34,1 anos; DP= 5,2), divididas em dois grupos: Grupo Creche (21 crianças que frequentavam a creche) e Grupo Não Creche (23 que não frequentavam). Avaliou-se o desenvolvimento aos 6, 12 e 18 meses da criança e a interação mãe-criança aos 12 e 18 meses. Os resultados revelaram que a frequência à creche não interferiu na qualidade da interação mãe-criança e não diferenciou os grupos quanto ao desenvolvimento infantil (cognitivo, linguagem, socioemocional), indicando que famílias de classes média e alta e creches com qualidade suficiente podem se igualar em seus eventuais benefícios.


Abstract The present study investigated the impact of childcare centers on mother-child interaction and on child development in the first two years of a child's life. The study was comprised of 44 children (M=6.4months, SD=1.6) and their mothers (M=34.1years, SD=5.2) divided into two groups: Childcare Center Group (N=21) and Non-Childcare Center Group (N=23). The infant development was evaluated at 6, 12 and 18 months and mother-child interaction was assessed at the 12th and 18th months. The results revealed that childcare centers did not affect the quality of mother-child interaction and did not set apart the groups in relation to child development (cognitive, language, socioemotional), indicating that middle and upper class families and qualified childcare centers can be equally beneficial.

Objective@#To analyze the risk factors of adverse neurodevelopmental prognostic in very /extremely low birth weight infants (VLBWI/ELBWI).@*Methods@#The 24 hours VLBWI/ELBWI admitted to the neonatal intensive care unit between January 2016 and October 2016 were enrolled.These infants were followed up and neurodevelopmental evaluation was performed at a corrected age of 12 months by using the Bayley Scales of Infant Development Ⅱ (BSID-Ⅱ). According to the neurodevelopmental outcomes, they were divided into normal and abnormal neurodeve-lopment groups.The data of prenatal, intrapartum and postpartum periods in the two groups were compared.Potential risk factors of neurodevelopmental impairment were analyzed with Logistic stepwise regression.@*Results@#A total of 167 VLBWI/ELBWI were enrolled, among which 14 cases died during hospitalization or after giving up treatment.At the corrected age of 12 months, 140(91.5%) infants completed follow-up and 13(8.5%) were lost to follow-up.Among 140 infants who completed neurodevelopmental evaluation at the corrected age of 12 months, there were 86 males, 54 females, and in which 28 cases had extremely low birth weight, 112 cases with very low birth weight, with gestational age of (30.4±2.2) weeks (25.3-36.0 weeks). There were one hundred and twenty-five cases with normal neurodevelopmental outcomes and 15 cases with abnormal neurodevelopmental outcomes.The results of univariate analysis showed that birth weight<1 000 g, exposure to antenatal steroids, N -terminal brain natriuretic peptide (NT-proBNP) ≥35 000 ng/L and bronchopulmonary dysplasia (BPD) in the two groups were statistically significant(P<0.05). Logistic stepwise regression showed that NT-proBNP level≥35 000 ng/L may independently predict neurodevelopmental impairment among VLBWI/ELBWI (OR=22.774, 95%CI: 3.079-168.425, P=0.002). Exposure to antenatal steroids may be a protective factor for neurodevelopmental impairment (OR=0.125, 95%CI: 0.020-0.782, P=0.026).@*Conclusions@#Plasma NT-proBNP level ≥35 000 ng/L may independently predict neurodevelopmental impairment among VLBWI/ELBWI.Antenatal corticosteroids may be the protective factor of poor neurodevelopmental outcomes.

Introdução: Crianças prematuras tendem a apresentar atrasos no desenvolvimento neuropsicomotor devido à imaturidade e à propensão de lesões no sistema nervoso central. Objetivo: Descrever o perfil epidemiológico e motor de crianças atendidas no follow-up da Maternidade Balbina Mestrinho (MBM) em Manaus/AM, verificando a associação entre diferentes fatores socioambientais e clínicos com o desenvolvimento motor (DM). Métodos: Foram avaliadas 25 crianças acompanhadas no follow-up da MBM, por meio da Escala Motora Infantil de Alberta e aplicação de questionário estruturado contendo dados clínicos e epidemiológicos. Resultados: Foi detectado que todas as crianças eram prematuras e 44% apresentaram atipicidade no DM, relacionada principalmente a idade corrigida (p=0,015) e ao grau de escolaridade materna ( p=0,019). Conclusão: O elevado índice de atipicidade no DM pode estar associado ao perfil amostral, cuja prematuridade infere em fragilidade de seus sistemas. Assim, sugere-se que maiores investigações sejam realizadas, a fim de relacionar outros fatores com o DM.


Introduction: Premature children tend to have neuropsychomotor development delays due to immature and propensity of lesions in the central nervous system. Objective: To describe the epidemiological and motor profile of children attended in the follow-up of Maternity Hospital Balbina Mestrinho in Manaus/AM, identifying possible motor development delays and correlating them with risk factors. Methods: 25 premature child were evaluated through the Alberta Infant Motor Scale and a structured questionnaire was used with clinical and epidemiological data from the child and parentes. Results: It was detected that 44% of the children presented atipical motor development, related to age corrected (p=0,015) and the degree of maternal schooling (p=0,019). Conclusion: The high index in atypical motor development can be associated whit the sample profile, whose the prematurity infers in the fragility of its systems. Thus, it is suggested that further investigations be performed in order to relate other factors to motor development.

Abstract: Introduction: Early identification of developmental delays or disabilities in children is a challenge and a global concern. In Mexico, the prevalence of childhood disability is 6%, and it is estimated that 25% of the unattended delays will have consequences on their potential. VANEDELA is a rapid screening test to detect and prevent developmental disorders in primary health care. Objective: To determine the external validity and test-retest reliability of the behavioral (DB) and reflex (DR) developmental formats of the VANEDELA screening test, compared with the diagnostic test of Gesell's Developmental Schedule Test. Method: Descriptive, transversal, and prospective study in 379 infants aged one to 24 months attending the Neurodevelopment Monitoring Laboratory / INP and the ISSSTE Tlalpan Family Medicine Clinic from 2011 to 2014. Measurements: A set of four aspects was evaluated including validity by external criteria: sensitivity, specificity, positive-negative predictive criteria, and test-retest reliability. Results: In the DB format, we found a 79% - 89% sensitivity (S) and a 83% - 95% specificity (Sp). In the DR format, a 18% - 35% sensitivity and a 81% - 96% specificity were found. Using both DB-DR formats, we found a 82% - 89% sensitivity and 72% to 91% specificity, and a test-retest reliability .62-1. Discussion and conclusion: The VANEDELA increased its validity parameters with respect to the previous assessment with adequate stability. The DB formats and the DB-DR combination of VANEDELA have an adequate validity based on external criteria, making them suitable for use in primary health care.


Resumen: Introducción: La identificación temprana de retrasos o discapacidades del desarrollo en los niños es un reto y una preocupación mundial. En México, la prevalencia de discapacidad infantil es del 6% y se estima que un 25% de los retrasos no atendidos tendrán consecuencias en su potencial alcanzado. El VANEDELA es una prueba de tamizaje rápido para detectar y prevenir alteraciones del desarrollo en el primer nivel de atención. Objetivo: Determinar la validez (criterio externo) y confiabilidad (test-retest) de los formatos de conductas (CD) y reacciones del desarrollo (RD) de la prueba de tamizaje VANEDELA, contrastado con la Prueba Diagnóstica de Desarrollo de Gesell. Método: Estudio descriptivo, transversal y prospectivo en 379 lactantes de uno a 24 meses que asistieron al Laboratorio de Seguimiento del Neurodesarrollo del INP y la Clínica de Medicina Familiar Tlalpan del ISSSTE de 2011 a 2012. Mediciones: Validez por criterio externo, sensibilidad y especificidad, validez predictiva positiva y negativa; confiabilidad test-retest (intervalo de siete días). Resultados: En el formato CD, se encontró sensibilidad (S) de 79% a 89% y especificidad (E) de 83% a 95%. Utilizando ambos formatos CD-RD, se encontraron una sensibilidad de 82% a 89% y una especificidad de 72% a 91%. Test-retest .62-1. Discusión y conclusión: El VANEDELA aumentó sus parámetros de validez con respecto a la valoración anterior, estabilidad adecuada. Los formatos CD y la combinación CD-RD del VANEDELA poseen una adecuada validez por criterio externo y estabilidad test-retest, para ser utilizada en el primer nivel de atención.

O presente estudo objetivou conhecer e analisar as concepções parentais acerca da intencionalidade comunicativa de bebês aos 3 e 6 meses do primeiro ano de vida. Foi utilizado um delineamento longitudinal, com a participação de 20 mães e de 20 pais, entrevistados em duas etapas. As entrevistas foram transcritas de forma literal e, em seguida, analisadas qualitativamente. Foram identificadas mais semelhanças do que diferenças entre os discursos maternos e paternos. Constatou-se que os relatos parentais apresentaram concepções que comportamentos infantis específicos podem ser indícios da habilidade de comunicação intencional. Especialmente na segunda etapa, quando os bebês tinham 6 meses de idade, os pais referiram comportamentos infantis que seriam exibidos para expressar vontades e indicar intenção. O conhecimento dessas concepções é importante para a compreensão das relações iniciais estabelecidas entre pais e seus filhos. (AU)


This study aimed to identify and analyze parental conceptions of the communicative intent of infants at 3 and 6 months in the first year of life. A longitudinal design was used, including 20 mothers and 20 fathers who were interviewed in two stages. The interviews were literally transcribed and then qualitatively analyzed. It was identified more similarities than differences between maternal and paternal speeches. It was found that parental reports presented conceptions that specific child behavior may be evidence of intentional communication skills. Especially in the second stage, when infants were 6 months old, parents mentioned child behavior that seemed to express wishes and indicate intention. Knowledge of these concepts is important for understanding the early relations between parents and their children. (AU)


Este estudio tuvo como objetivo conocer y analizar las concepciones de los padres sobre la intención comunicativa de los bebés a los 3 y 6 meses en su primer año de vida. Se utilizó un delineamiento longitudinal, y participaron 20 madres y 20 padres entrevistados en dos etapas. Las entrevistas fueron transcriptas literalmente y analizadas cualitativamente. Fueron identificadas más semejanzas que diferencias entre los discursos maternos y paternos. Se constató que los informes de los padres presentan concepciones sobre el hecho de que, comportamientos infantiles específicos, pueden ser indicios de la habilidad de comunicación intencional. Especialmente en la segunda etapa, cuando los bebés tenían 6 meses de edad, los padres indicaron que comportamientos infantiles fueron exibidos para expresar deseos e indicar la intención. El conocimiento de esas concepciones es importante para la comprensión de las relaciones iniciales estabelecidas entre padres y sus hijos. (AU)

Abstract Introduction: The motor trajectory of pre-term children is an important indicator of health during infancy, since alterations may be a signal for the need of professional intervention. Objective: To describe percentiles and motor development curves for Brazilian preterm infants in the first year of life, determining the reference values for categorization of motor performance assessed by the AIMS. Methods: Participated in this cross-sectional study 976 children born pre-term, newly-born to 12 months of corrected age. The Alberta Infant Motor Scale (AIMS) was used to assess participants' motor development. The scores of the Brazilian norms were used as comparison criteria. Results: Children born pre-term showed lower scores compared to children born full-term indicating the need for a specific percentile curve for that population. The scores differentiated at P1 to P99 percentiles allowing for the categorization of children with typical development, at risk and with atypical development. At 0, 4, 8, 9, 10, 11 and 12 months an overlapping of extreme percentiles (P1, P5 and P10; P90, P95 and P99) was observed, but not in the other percentiles. Conclusion: The percentiles described indicate that preterm children presented lower motor performance than full-term children and AIMS has discriminant power for the clinical evaluation of these children. The developmental curves showed lower capacity for behavioral differentiation in the extreme percentiles.


Resumo Introdução: A trajetória motora de uma criança nascida pré-termo é um importante indicador de saúde infantil, uma vez que alterações podem ser um alerta da necessidade de intervenção profissional. Objetivo: Descrever os percentis e as curvas do desenvolvimento motor para prematuros brasileiros no primeiro ano de idade, determinando os valores de referência para categorização do desempenho motor avaliado pela AIMS. Métodos: Estudo transversal no qual participaram 976 crianças prematuras, recém-nascidos até 12 meses de idade corrigida. A Alberta Infant Motor Scale (AIMS) foi utilizada na avaliação do desenvolvimento motor dos participantes. Os escores normativos da AIMS no Brasil foram utilizados como critério de comparação. Resultados: Crianças prematuras demonstraram escores mais baixos do que crianças nascidas a termo, indicando a necessidade das curvas de percentil específicas para esta população. Os escores se diferenciaram nos percentis P1 a P99, permitindo a categorização de crianças típicas, suspeitas e atípicas. Aos 0, 4, 8, 9, 10, 11 e 12 meses de idade, observou-se sobreposição nos percentis extremos (P1, P5 e P10; P90, P95 e P99) mas não nos demais percentis. Conclusão: Os percentis descritos indicam que os prematuros apresentaram desempenho motor inferior a crianças a termo e a AIMS possui poder discriminante para avaliação clínica destas crianças. As curvas de desenvolvimento demonstraram menor capacidade de diferenciação comportamental nos percentis extremos.

Objetivo: Describir los percentiles y las curvas del desarrollo motor para prematuros brasileños en el primer año de edad, determinando los valores de referencia para categorización del desempeño motor evaluado por la AIMS. Métodos: Estudio transversal en el que participaron 976 niños prematuros, recién nacidos hasta 12 meses de edad corregida. La Alberta Infant Motor Scale (AIMS) fue utilizada en la evaluación del desarrollo motor de los participantes. Los escores normativos de la AIMS en Brasil se utilizaron como criterio de comparación. Resultados: Niños prematuros mostraron escores más bajos que los niños nacidos a término, indicando la necesidad de las curvas de percentil específicas para esta población. Los escores se diferenciaron en los percentiles P1 a P99, permitiendo la categorización de niños típicos, sospechosos y atípicos. A los 0, 4, 8, 9, 10, 11 y 12 meses de edad, se observó superposición en los percentiles extremos (P1, P5 y P10, P90, P95 y P99), pero no en los demás percentiles. Conclusión: Los porcentajes descritos indican que los prematuros presentaron desempeño motor inferior a niños a término y la AIMS posee poder discriminante para la evaluación clínica de estos niños. Las curvas de desarrollo demostraron menor capacidad de diferenciación conductual en los percentiles extremos.

OBJETIVOS: Avaliar a aquisição de marcos motores nos primeiros dois anos de vida e as relações entre motricidade ampla e motricidade fina, cognição e linguagem em lactentes que frequentam creches. MÉTODOS: Estudo transversal de uma amostra não probabilística de lactentes frequentadores de creches públicas e privadas de uma cidade da região sul do Brasil. Foram utilizadas a Alberta Infant Motor Scale e a Bayley Scales of Infant and Toddler Development para a avaliação do desempenho motor de cada criança. Utilizou-se estatística descritiva para a caracterização da amostra e regressão linear simples em três distintas faixas etárias (grupo 1: entre seis e nove meses; grupo 2: entre 10 e 12 meses; grupo 3: entre 13 e 16 meses) para as análises das relações entre as variáveis motricidade ampla e fina, cognição e linguagem. RESULTADOS: Foram estudados 63 bebês entre seis e 16 meses de idade que não apresentaram diferenças estatisticamente significativas nas suas características biológicas e sociodemográficas, quando comparados os três grupos estratificados. A maioria das crianças não adquiriu os marcos motores nos períodos esperados para a faixa etária. Houve correlação entre os escores de desenvolvimento, e observou-se que a motricidade ampla foi capaz de explicar grande parte da variabilidade nos escores de motricidade fina, cognição e linguagem (R2 ajust>0,5) ao longo dos dois primeiros anos de vida. CONCLUSÕES: As habilidades motoras observadas ocorreram, em geral, tardiamente na amostra observada. O atraso na aquisição dos marcos motores amplos repercutiu no atraso global do desenvolvimento, corroborando com a influência da motricidade ampla nos demais domínios do desenvolvimento.


AIMS: To evaluate the acquisition of motor milestones in the first two years of life and the relationship between gross motor function and manipulation, cognition and language in infants who attend day care centers. METHODS: Cross-sectional study with a non-probabilistic sample of infants attending public and private day care centers in a city in southern Brazil. The Alberta Infant Motor Scale and the Bayley Scales of Infant and Toddler Development were used to assess the motor performance of each child. Descriptive statistics were used for the characterization of the sample and simple linear regression in three different age groups (group 1: between six and nine months; group 2: between 10 and 12 months; group 3: between 13 and 16 months) for the analysis of relationships between the variables gross and fine motor skills, cognition and language. RESULTS: We studied 63 infants between six and 16 months of age who did not present statistically significant differences in their biological and sociodemographic characteristics when the three stratified groups were compared. Most children did not acquire the developmental milestones in the period expected for their age group. There was correlation between the developmental scores, and the gross motor function was able to explain a large part of the variability in the fine motor function, cognition and language scores (R2 ajust>0.5) over the first two years of life. CONCLUSIONS: The evaluated motor skills occurred generally late in the sample. The delay in the acquisition of gross motor milestones had an impact on the overall developmental skills, corroborating the influence of gross motor function in the other domains of development. Tem como objetivo narrar as reflexões que descrevem minha trajetória pessoal e profissional, semeada durante estes 30 anos, desde minha graduação em terapia ocupacional. Descrevo os caminhos percorridos com determinação e empenho, e também os acasos que desviam as rotas, mas apresentam grandes oportunidades de aprendizagem. Tais caminhos foram semeados com práticas, ensino e pesquisa na atenção à criança e o papel da terapia ocupacional, os quais deram como frutos a formação de diversos terapeutas ocupacionais, algumas produções científicas, contribuindo para esta área de conhecimento e, mais recentemente, a formação de pós-graduandos, em dois programas de pós-graduação distintos, que reúnem, em nosso grupo de pesquisa, além de terapeutas ocupacionais, outros profissionais de áreas afins. Introducción: dentro de las alteraciones del neurodesarrollo, la prematurez es un factor de riesgo. Los niños prematuros tempranamente pueden presentar retraso en la comunicación y lenguaje, comparados con la población general. Objetivos: Dar a conocer la influencia de la prematurez en el desarrollo de la comunicación y el abordaje posible a realizar. Material y métodos: Análisis de 59 historias clínicas de niños prematuros que asistieron a control y seguimiento en el Gabinete de Intervención Temprana del Hospital Argerich, desde enero de 2011 hasta enero de 2017; y Escala Bayley del Desarrollo Infantil aplicada a cada niño, contemplando habilidades comunicativas y lingüísticas. Resultados: De las habilidades comunicativas esperadas para cada rango de edad, los diversos grupos (6, 9, 12, 18 meses de edad corregida) no alcanzaron a completar el 50% de las mismas. Analizando los resultados de cada grupo de prematuros (Extremo, Muy, Moderado y Tardío), se encontró que a medida que aumenta la edad gestacional existe un aumento de porcentaje de puntuación menor a dos desvíos estándar, visualizado a partir de los 9 meses de edad corregida, presentando compromiso cognitivo general y no sólo del lenguaje. Conclusiones: La prematurez afecta el desempeño comunicativo de los niños. La intervención fonoaudiológica temprana y el desarrollo de dispositivos de estimulación de comunicación y lenguaje contribuirían a disminuir el impacto que puede tener en el desarrollo global de los niños, favoreciendo la intra e intercomunicación, desde los primeros meses de vida.


Introduction: within neurodevelopmental disorders, prematurity is a risk factor. Also, premature infants, may have delays in language and communication acquisition compared to the general population. Objectives: Present the influence of prematurity in the development of communication and the possible approach to be performed. Material and methods: Analysis of 59 clinical records of premature infants, who attended control and follow-up at the Argerich Hospital Early Intervention Service, from January 2011 to January 2017; And Bayley's Scale of Child Development applied to each child, contemplating communicative and linguistic skills. Results: Of the communicative skills expected for each age range, the various groups (6, 9, 12, 18 months of age corrected) fail to complete 50% of the same. Analyzing the results of each group of premature infants (Extreme, Very, Moderate and Late), it was found that as gestational age increases, there is an increase in percentage of score less than two standard deviations observed after 9 months of age corrected, presenting general cognitive commitment and not just of the language. Conclusions: Prematurity affects the communicative performance of children. Early speech therapy and the development of language and communication stimulation devices, would contribute to reduce the impact that can have on the global development of children, favoring intra and intercommunication, from first months of life.


Introdução: dentro das alterações do neurodesenvolvimento a prematuridade é um fator de risco. Os prematuros podem ter atrasos na aprendizagem da linguagem, em comparação com a população em geral. Objetivos: Apresentar a influência da prematuridade no desenvolvimento da comunicação e sua possível abordagem. Material e métodos: Análise de 59 histórias clínicas de infantes prematuros que participaram do monitoramento e acompanhamento no gabinete de intervenção precoce do Hospital Argerich, de janeiro de 2011 à janeiro de 2017; a escala Bayley de crianças em desenvolvimento aplicada a cada criança, contemplando as habilidades comunicativas e lingüísticas. Resultados: Entre as habilidades comunicativas esperadas para cada facha de etária, os diversos grupos (6, 9, 12, 18 meses de idade corregida) não chegam a completar 50% dessas habilidades. Analisando os resultados de cada grupo de prematuros (extremo, muito, moderado e tarde), encontramos que a medida que aumenta a idade gestacional existe um aumento da porcentagem na pontuação menor a dois desvios estândar, observado a partir dos 9 meses, apresentando compromisso cognitivo general e não somente da linguagem. Conclusões: A prematuridade afeta o desempenho comunicativo de crianças. A intervenção fonoaudiológica precoce e o desenvolvimento de dispositivos de estimulação da linguagem, ajudaria a diminuir o impacto que pode ter sobre o desenvolvimento global das crianças, favorecendo a intra e intercomunicação a partir dos primeiros meses de vida.
